Support coordination is a crucial component of the NDIS designed to assist individuals with disabilities in understanding and utilizing their plans effectively. A support coordinator acts as a liaison between the participant, their families, and service providers, ensuring that support needs are met and goals are achieved.
There are three levels of support coordination:
- Support Connection: This level focuses on building your capacity to connect with informal, community, and funded supports.
- Support Coordination: This involves assisting you in understanding and using your plan, connecting with providers, and building skills to coordinate your supports.
- Specialist Support Coordination: This level is for individuals with complex situations requiring specialized support to manage challenges and ensure consistent service delivery.
