TeamHEALTH – Rent to Live Program Services

Transitional housing supporting individuals and families with skills, stability and support to secure long-term housing.

The Rent to Live Program, funded by the Department for Children and Families, provides supported transitional accommodation in the Greater Darwin region, alongside personalised tenancy and recovery support and the rental history needed to successfully move into long term housing.

Support is delivered by experienced workers, with each participant working towards and individual exit strategy and the confidence to independently manage their own tenancy.

Support is provided for a duration suited to each participant’s needs and goals, typically ranging from 6-12 months. Progress is regularly reviewed, and the focus is always on ensuring participants are ready and equipped to successfully manage their own tenancy before transitioning, with planned aftercare support available after moving on.

Eligibility

The Program is open to individuals and families who meet the following criteria:

  • Individuals aged 18 years or over, or families
  • Living in the Greater Darwin Region
  • Currently experiencing or at risk of homelessness
  • Intending to transition into long term rental market
  • Facing barriers to obtaining or maintaining private rental housing due to social, personal or systemic factors

What the Program Offers

Rent to Live combines accommodation with practical, tenancy-focused support to give participants the best chance of securing and maintaining a home in the community.

The program includes:

  • Transitional accommodation – Supported accommodation options across Greater Darwin, providing a safe and stable base during the program
  • Tenancy and recovery support – Culturally secure, structured occupancy and recovery support, tailored to your pace and goals
  • Individual recovery and capacity building plans – Personalised plans with regular goal review and planning focused for long-term, stable housing outcomes
  • Warm referrals to services – Supported connections to health, employment, education and community services, as needed throughout the program
  • Up to 3 months aftercare – Support following participant’s transition into long-term housing in the community.
