    With a focus on physical, social and emotional well-being combined with advice and support in maintaining safe and health homes we assist people through:

    • Domestic assistance

    • Financial counselling and advice
    • Accessing other community based support services
    • Rubbish removal • Advocacy and referral services

    TeamHEALTH – The Pathways Home & Stronger Foundations Programs

    Pathways Home and Stronger Foundations are medium-term supported transitional housing programs for men experiencing or at risk of homelessness.

    Stronger Foundations is open to men aged 18 or more. Pathways home is specifically dedicated to adult men who are transitioning from custodial corrections and are at risk of homelessness.

    The program offers structured shared accommodation combined with tenancy and recovery support. Participants receive tenancy and recovery-focused support for 6-12 months, with planned transition into long-term housing and aftercare support available.

    In house staff are available 7 days per week to provide structured tenancy and recovery support.

    These programs are funded by the Department of Children and Families NT.

    Eligibility

    Eligible participants include men who:

    • Are aged 18 years or over
    • Are willing to live in a shared accommodation environment
    • Are open to engaging in structured support
    • Are currently at risk of or experiencing homelessness
    • Pathways Home is only available to men experiencing or at risk of homeliness as part of exiting the custodial corrections system.

    What The Program Offers

    These programs include:

    • Supported transitional accommodation in shared houses across Greater Darwin with onsite staff
    • Structured occupancy and recovery support for a period of 6-12 months
    • Shared household responsibilities where participants contribute to maintaining the home and engaging in shared-house meetings and decision-making
    • Individual recovery and capacity building plans with regular goal review and planning for long-term housing
    • Warm referrals to health, cultural support, employment, education etc as needed
    • Up to 3 months aftercare support following transition into longer term housing
    • For Pathways home – Support to navigate post-release requirements (where applicable) including practical support to maintain compliance and coordinate with relevant services
